SAGE Dining Services

Notre Dame is proud to partner with SAGE Dining Services®. SAGE, established in 1990, is the leading food service provider for independent schools and colleges throughout North America. SAGE's unparalleled expertise in nutrition, sustainability, and culinary trends produces exceptional dining experiences that delight the senses, inspire minds, and foster community.

SAGE provides our school community with customized, delicious, dietitian-approved meals, cooked from scratch using fresh, locally sourced ingredients. Their talented chefs enhance mealtime with a wide array of options and flavors, including authentic Asian dishes, Latin American cuisine, and vegetarian and vegan fare. A monthly Educational Seasonings program explores food in cultural and historical contexts, and chefs often engage communities with exciting display cooking.

Health Forms

Students must submit the following to the Health Office prior to the first day of school*:

1. Physical Exam
  • State law requires 6th and 11th graders to submit a physical exam dated no earlier than one year prior to the first day of school. 
  • New students entering any grade must also submit a recent physical examination. 
  • No specific form is required for these exams; however, the form submitted must be an original.
2. Immunization Record3. Dental Form (Grade 7 only)
*Please note that, per Notre Dame Policy, students will not be allowed to begin the school year without required health form documentation.

Uniforms

The uniform consists of a kilt (purchased from Flynn & O'Hara) and a golf shirt (purchased from Lands End, Flynn & O'Hara, or the school store). The winter uniform in effect from November to April includes a school sweatshirt (purchased in the school store) and blue tights. There is no school shoe. Students may wear traditional school shoes in brown, black, tan, or navy or conservatively-colored sneakers.

"Shamrock Shop" School Store

The ND Shamrock Shop is open Monday through Friday during the school year. Please see the school website for specific hours. The store is also open for select events on campus throughout the year.
